The Living-at-Home program of Senior Services, Inc., provides case management through the Community Alternatives Program for Disabled Adults (CAP-DA).  CAP-DA is a Medicaid program providing a coordinated network of support services in the home that allows the aged or disabled adult to remain at home for as long as possible, or to return home from a stay at a medical facility rather than going into institutional care.  

 

 

Services provided

Living-at-Home provides in-home evaluation of clients’ abilities, needs and circumstances by a registered nurse. Close, ongoing supervision of clients is then provided by case managers assigned to oversee their care.

 

The following services are provided on an as-needed basis:

 

          • In-home aide services such as bathing and

            dressing

 

          • Nursing visits through a home health agency

 

          • Adult day health care at approved centers

 

          • Home mobility aids, including wheelchair ramps

 

          • Telephone alert

 

          • Incontinence supplies

 

          • Nutritional supplements

 

          • Respite care

Eligibility

The Community Alternatives Program is available to elderly (65 and older) and disabled (18 and older) adults.   The program has specific income requirements, and the cost of services provided must be less than the cost of a nursing facility.  A participant must be approved for an intermediate or skilled level of care, and his or her health, safety and well-being must be assured in the home setting.
